Registering with the Surgery
Our practice list is open and we are accepting new patients that live in our practice area. If you are a Temporary Resident please see below.
New Patients:
All registration forms can be downloaded from the 'Registration Forms' section on this page [See Top Right]. They are also available from reception.
If you would like to register at this practice:
- Please complete a registration form for each family member.
- All patients need to bring their personal medical card with the above registration forms. If you do not have your medical card, please fill in our permanent patient registration form (GMS1).
- Once you have completed the forms above please drop them off at the surgery. Once we receive your forms we can book you a new patient appointment at the surgery with our health care assistant. We will measure your height, weight and blood pressure during your new patient appointment.
- Please note if you are registering a child below the age of 7 years old please ensure that you bring a copy of the child's red book or immunisation history, so that we can take a copy for our records.
- If you have not been registered with a GP before please bring proof of your address (bill or bank statement) and photo identity (passport/driving license).
Temporary Residents:
The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.
To register you will need to complete our Temporary Resident Registration form which is available from reception, or can be downloaded from the 'Registration Forms' section of this page [See right]:
